What to Expect at Your Visit
Here’s what to expect from the moment you book your appointment to the moment you walk in.
After you book
- You’ll get a confirmation text with your date, time, and clinic location
- We’ll send reminders by text leading up to your visit
- If there’s paperwork to complete ahead of time, we’ll text it to you or have it ready at check-in
- We’ll let you know ahead of time if you need anything else, like a referral, recent imaging, or records from another provider
- Plans change — just call us to reschedule or cancel
At your appointment
- New patients: please plan to arrive 15 minutes early so check-in is done before your scheduled time
- Your care team will talk with you about what brought you in today and what you’re hoping to walk away with — whether that’s answers, relief, or just a check-up
- Together, you’ll build a plan that’s right for you, drawing on our full team of specialists when it helps
What to bring
- Photo ID and insurance card(s)
- A current list of medications and allergies
- Any related records or imaging, if you have them
- Your copay or preferred payment method (financial assistance is available for those who qualify)

Learning your options about insurance coverage can be overwhelming. We have Eligibility Specialists at main locations in Springdale, Rogers, Siloam Springs, and Fayetteville who can help you enroll in an insurance plan.

Discount Programs
We believe that quality, comprehensive health care should be available to everyone. That’s why we offer all of our services on a sliding fee scale according to each patient’s financial situation, determined by income and household size. To determine what discount you are eligible for, please bring Proof of Income (POI) to your first appointment.

Good Faith Estimate
You Have the Right to Receive a Good Faith Estimate Explaining How Much Your Medical Care Will Cost
- Under the law, healthcare providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
- Make sure your healthcare provider gives you a Good Faith Estimate in writing at least one business day before your medical service or item. You can also ask your healthcare provider for a Good Faith Estimate before you schedule an item or service.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
- Make sure to save a copy or picture of your Good Faith Estimate

Federal Tort Claims Act (FTCA) Coverage
Community Clinic is deemed a Public Health Service employee under 42 U.S.C. § 233(g)-(n). As a result, Community Clinic and its covered individuals are provided liability protection through the Federal Tort Claims Act (FTCA).
